A medical provider will spend 1-2 hours conducting a comprehensive medical and behavioral history, including a physical exam and developmental assessment.
A medical provider will meet with the family and give feedback about developmental functioning and suggested behavior modifications. At this time, if required, we will consider if blood tests for genetic conditions or a brain Magnetic Resonance Imaging is needed.
With the family’s input, our clinic will help facilitate referrals to other clinicians, such as speech pathologists or psychologists, if needed.
Sometimes, medication management for behavioral disorders is prescribed at this time.
If a psychological evaluation is needed, children are referred to psychologists within the clinic or sometimes, to an additional source within the community.
Psychologists will conduct a more in depth behavioral health history of the child and family. This consists of a standardized evaluation in which questions are asked of parents about independent living and a formal evaluation of the child’s developmental ability or intelligence.
There will be a follow up session so that the psychologists can give feedback about the evaluation and recommendations for educational and behavioral changes.
Preparing for your visit:
We ask that all families have their primary care doctor fax a referral form. Links to the referral form are below. At this time they may call our office for a referral form.
When we receive the referral, we will mail a packet for parents to complete.
It is important that your child gets a restful night’s sleep and eats breakfast on the day of the appointment as he/she will have a busy day. Appointments with our Eastern Shore Developmental Clinic medical provider will last for about 1-2 hours. Appointments with our psychologists will last 2 1/2 hours.
Please arrive 15 minutes before your scheduled appointment time for paperwork. If a family arrives 20 minutes after the scheduled time or later, their child will not be seen, and they will need to call the clinic to reschedule.
Please bring the following items:
- Insurance cards
- Copies of medical information
To schedule or cancel an appointment, call 251-625-2400.
Our office hours are Monday – Thursday 8 a.m. to 5 p.m.
We also ask that in addition to the completed new patient packet of information, you include your child’s IEP (individualized Education Plan) or IFSP (Individual Family Service Plan) before the visit if you have one.
